    Norah O’Donnell: What I learned from my doctor about menopause

    For this week’s “Healthful,” I sat down with my very own doctor, Rachel Rubin, to tackle the important topic of menopause: How your body changes, why women may be needlessly suffering, and what you can actually do about it.

    Here’s what I learned in our conversation, streaming now on YouTube and available wherever you listen to your podcasts:

    Menopause as “a castration event”

    “Menopause is a castration event, whether we like it or not, whether it’s natural or not,” Dr. Rubin said. “Whether it’s supposed to happen or not, we are outliving our ovaries.”

    Rubin described menopause as essentially the ovarian function turning off: Your ovaries no longer make estrogen, progesterone or testosterone. “You’re not making the hormones that you were during your reproductive years, and that has consequences because you have hormone receptors throughout your entire body,” she explained.

    The severity and symptoms are different for everyone, but women commonly experience hot flashes, poor sleep, mood changes, weight gain and memory issues.

    While Rubin said some men see a natural decline of testosterone as they age, they “don’t really get to ‘castrate’ levels.” She explained her castration remarks in the context of a gas tank analogy: “Women, their gas tank is zero. Men, they kind of live at like half a tank, and maybe if they get to a quarter of the tank, they can go to their doctor, they get some testosterone, and they go back to half to three-quarters tank and they feel great.”

    Vaginal hormones can prevent UTIs — and save billions in Medicare

    Urinary tract infections are common during and after menopause, and if untreated, can lead to sepsis, hospitalization and even death. It’s why Rubin said she thinks all menopausal women should be offered vaginal hormones, which fix the urinary tract’s microbiome and can prevent UTIs.

    Right now, fewer than one in 10 postmenopausal women are using vaginal estrogen, even though its use is supported by the American Urological Association, and it’s often covered by insurance.

    Rubin said giving women vaginal estrogen could save Medicare $6 billion to $22 billion a year, citing costs around urgent care visits, hospitalizations and ICU admissions.

    Women actually produce more testosterone than estrogen

    You need testosterone to make estrogen in the ovaries — and women make more testosterone than estrogen.

    Rubin said there’s global consensus that testosterone helps menopausal and perimenopausal women with low libido, but there’s no FDA-approved testosterone product for women. Although, it can be prescribed off-label. Rubin said her patients who take testosterone tell her it helps with more than libido — that patients come back and say, “I just feel more like me.”

    I’ve never talked about this publicly before, but I use testosterone prescribed by Rubin — it comes in a tube that’s normally prescribed to men, but I administer it in much smaller doses. There wasn’t an “aha” moment for me, but I noticed its effects over time, and in concert with other hormones.

    US embassies in the Middle East prepare for extended period with reduced staff amid Iran war

    The State Department is asking US embassies in the Middle East to create plans to continue operating with a small number of staff on the ground, sources told CNN, as the war with Iran shows no signs of resolution.

    Additionally, personnel who have been displaced from their posts in the Middle East are increasingly being given the option to curtail their assignments, the sources said.

    The plans have not been finalized, the sources said, and it is unclear if they will be implemented at all of the embassies that are currently on reduced staffing. Still, the developments underscore that the State Department does not expect to return to normal staffing in the region soon amid the looming threat of a full-scale return to war.

    A State Department spokesperson told CNN that they “do not discuss internal deliberations or post-specific contingency planning,” but noted that the department “continually reviews the security and staffing posture at every diplomatic mission based on conditions on the ground and adjusts personnel levels as appropriate.”

    “The safety and security of our personnel and their families remains the Department’s top priority as we continue to assess conditions across the region,” the spokesperson said. “Decisions regarding the status of any post are made based on a range of security and operational factors, in close coordination between posts and Washington.”

    “Personnel matters, including individual curtailment requests, are handled on a case-by-case basis,” they added.

    The State Department ordered nonemergency personnel and family members to leave almost every diplomatic post in the region shortly after the war began in late February. That has led to nearly six months of uncertainty about whether the posts would be able to return to normal and all diplomats could return. The plans for “reduced operations,” once finalized, could provide some clarity to US diplomats and their families who have been displaced.

    Meanwhile, efforts to bring the war to an end have faltered. The memorandum of understanding between the two sides has collapsed. There was more than a week of back-and-forth strikes at the end of July. A renewed push for an agreement to fully reopen the Strait of Hormuz has yet to succeed.

    The State Department did not reduce staffing at most of its embassies in the region before the US and Israel began their military campaign. Ahead of the war, only Lebanon and Israel were in ordered and authorized departure status, respectively. Authorized departure means nonemergency personnel and family members could choose to leave but were not required to.

    Within weeks, as US diplomatic facilities across the region came under attack by Iran and its proxies, the department ordered nonemergency personnel and families to leave Bahrain, Iraq, Jordan, Qatar, Saudi Arabia and the United Arab Emirates. The US Embassy in Kuwait suspended operations entirely in March and only resumed emergency operations for Americans in late June. It remains under ordered departure. The US Embassy in Oman is under authorized departure.

    The sudden drawdown in staffing left diplomats and family members, many of whom had years left on their assignments, scrambling to find housing and to enroll their children in school back in the US. However, because it was unclear how long embassies would operate with reduced staffing, families couldn’t make longer-term commitments when they returned home. If normal operations resumed, they would be expected to quickly return to the Middle East.

    One diplomatic spouse said their family has been “hopping around to different housing situations this whole time,” because they couldn’t risk the potential financial repercussions of signing a long-term lease and breaking it.

    “We’ve had evacuations in various parts of the world, but this is different in that there’s such a large portion of people from one part of the world with so many people coming back to one spot,” they added.

    Those who were forced to leave the region left behind almost all of their belongings, as well as support networks of friends who can be critical to an overseas posting.

    The diplomatic spouse told CNN they feel “a tremendous sadness” for “the people we care about and the work that we’re part of” in the region.

    “We had no idea when we walked out of our house in March that we not only wouldn’t be returning in six months, but now the reality is, we won’t be returning at all,” they said, requesting anonymity due to concerns of retaliation.

    Under State Department rules, diplomatic posts cannot have their “ordered departure” status extended for more than 180 days. Once that limit is reached, if they are unable to return to normal status, they switch to “restricted operations,” which includes caps on in-country staffing. The State Department’s foreign affairs manual notes that restricted status “is intended as a temporary measure to address safety and security,” but “it may continue as long as necessary to ensure that adequate safety and security measures are in place.”

    For the posts in the Middle East, restricted operations status will likely mean no children and few spouses will be allowed to return, sources said.

    The State Department spokesperson said the department remains “committed to supporting our workforce and their families throughout this process and will provide updates through appropriate channels as decisions are made.”

    It will also mean a continued shortage of US diplomats on the ground. For those who are still not able to return to their posts, they may try to curtail, but it is not clear whether there are enough alternate jobs for them.

    And fewer diplomats on the ground could impact the State Department’s ability to provide quick consular assistance to Americans abroad and to advance the administration’s priorities, former diplomats said.

    “When you have fewer people, there’s less you can do, and there’s a lot that you cannot do remotely,” said John Bass, a former career diplomat who served as an under secretary of state for management.

    “The fewer diplomats you have in country, the fewer people you have who are focused on what is happening in that country, in that government, keeping tabs on key issues for the United States,” he explained. “You’ve got fewer people there to be promoting the US government’s views on what is happening and why, and what is in our interests and the interests of that country, working together, to try to solve a common problem.”

    He noted that leaders might discuss a broad agreement on a matter, but it comes down to the experienced diplomats on the ground “to be able to really get into the details with that host government about how to move forward.”

    Bass, who was an ambassador to Turkey, Georgia and Afghanistan, told CNN that the restricted operations status could potentially last “for months, if not years.”

    The State Department spokesperson disputed the idea that diplomatic efforts by the US have been “limited” by having fewer people on the ground.

    “We have seen sustained engagement from the highest levels of the Trump Administration with our partners in the Middle East, and our relationship with our allies in the region continues to get stronger,” they said.

    诈骗团伙最有可能在洛杉矶等大城市布设设备。但即便在小城镇,盗刷器也可能构成威胁。

    佩克表示,为保障自身安全,应尽量避免刷卡支付。政府救助卡通常只能刷卡使用,这使其处于不利地位。但对于有其他选择的消费者来说,使用免接触支付是“保护自己的最佳方式”。

    Many Americans may have encountered a predatory scam — and may not even know it.

    The U.S. Secret Service says the use of skimming devices, which capture account details and personal identification numbers from bank and government benefits cards, is on the rise. The devices can be set up in seconds on debit and credit card readers.

    CBS News joined the Secret Service, which investigates financial crimes as well as protecting the president, for a ride-along while agents searched for skimmers at hundreds of retailers across Los Angeles.

    Once the device is in place on a card terminal or ATM, it collects data every time a card is swiped. Often, a wireless transmitter hidden inside the skimmer sends the data to the scammer. Secret Service analyst Vince Porter told CBS News that officials believe a single skimmer can net up to $1 million in stolen funds.

    The devices are tough to recognize and harder to remove — in some cases, law enforcement has to pry the skimmer off, leaving residue behind. There’s no way to know how long a device has been in use, Porter said.

    In 2025 alone, the Secret Service removed more than 400 illegal skimming devices, which the agency said accounts for just a fraction of the devices out there. A CBS News investigation has found 32 active federal probes spanning 15 states.

    The agency and law enforcement partners have noticed an increase in skimming activity in the past two years. As the crime becomes more common, the losses increase. Skimming costs financial institutions and consumers more than $1 billion each year, according to Secret Service estimates.

    Special agent Michael Peck, the leader of the Secret Service’s criminal investigative division, said that groups of organized criminals from outside the country are responsible for the scam.

    The groups enter the U.S. from Eastern Europe, Asia and Central and South America, Peck said, with many crossing the U.S.-Mexico border to enter the country. One subject was apprehended after traveling down from Canada on a Jet Ski, Peck told CBS News.

    “A one-stop crime”

    Criminals often target stores that take EBT cards, which hold government benefit money. A single government assistance card can receive thousands of taxpayer dollars from federal and state programs, including food stamps and Section 8 housing payments, each month.

    Many government assistance cards have to be swiped, leaving them without the added security of tap-and-pay or a chip. When stores advertise acceptance of these funds, they can become a target, Porter said.

    “It’s a one-stop crime,” he said.

    Victims often have little recourse. Once the benefits are stolen, they can be nearly impossible to recover because of the number of agencies involved in distributing the funds.

    Mary Bowen, a mother of six who relies on assistance, said she can’t pay her rent or buy groceries because her benefits were stolen last week.

    “It just disappeared. I don’t even have cash in here,” Bowen said. “Literally, I’ve been getting food from my friends because they took everything. They took it all from me.”

    As part of this operation, law enforcement visited 328 businesses and inspected 1,749 devices in California. They seized 16 skimmers, preventing an estimated $16.6 million in fraud loss, according to the Secret Service. Similar operations have been carried out in dozens of U.S. cities.

    Scams are most likely to be found in major cities like Los Angeles. But even in small towns, skimmers can pose a threat.

    To stay safe, avoid swiping as much as possible, Peck said. Government benefit cards usually need to be swiped, putting them at a disadvantage. But for those who have the option, using tap-and-pay is “the best way to protect yourself as a consumer,” Peck said.

    The FBI said the enforcement effort has resulted in 6,121 arrests, 237 search warrants and the recovery of $252,592 in cash.

    Patel vowed that the federal enforcement effort would continue.

    “The greatest country on earth should have a capital city reflecting it — where American taxpayers can live, visit, and petition their government without fear of violent crime,” Patel said.

    The federal surge followed a series of high-profile violent crimes in Washington, including the June 2025 killing of 21-year-old congressional intern Eric Tarpinian, the beating of a former DOGE staffer and the fatal shooting of a 14-year-old boy in a case involving alleged MS-13 gang members.

    The National Guard deployment also drew criticism from Democratic leaders, including then-Washington, D.C., Mayor Muriel Bowser.

    Hospitals push for upfront payments as patient deductibles and out-of-pocket costs rise

    Thomas Zordani flew from his home in Denver to Phoenix for a consultation with a Mayo Clinic neurosurgeon, hoping to find out what could be done to treat his debilitating headaches after worrisome brain scan findings.

    When making the appointment, Zordani said, he’d been told the clinic was in his insurer’s network. Upon arrival, Zordani was summoned to the clinic’s financial office and told he had to make a $5,000 preservice deposit, because Mayo had since determined it did not accept his insurance. He was automatically designated “self-pay,” even though his plan had out-of-network benefits.

    Not having that kind of cash on hand — and angry on principle — he refused. His appointment was canceled.

    “I was so livid,” Zordani said, recalling that day in early April 2024. He later learned that Mayo had sent a message to him in his insurance carrier’s patient portal shortly before his visit with an estimate of the cost: $565, not the larger amount it later demanded.

    Traditionally, patients usually receive bills for their share only after getting treatment. But what Zordani faced is becoming increasingly common — hospitals or other medical providers seeking prepayments.

    “We regret that this individual’s experience did not meet the high standard of communication we strive to provide when helping our patients understand their insurance coverage and financial responsibility,” Andrea Kalmanovitz, Mayo’s communications director, said in an emailed statement. “When prospective patients don’t have clarity that Mayo Clinic is not in-network with their health plan, unexpected pre-service deposit requests may result.”

    Mayo’s website says it requires prepayments in a variety of cases, including for “noncontracted” — also known as out-of-network — insurance plans.

    The trend of hospitals asking for money up front represents a double whammy for patients.

    Medical providers are collecting larger shares of what patients might owe at a time when rising deductibles mean patients are owing more for care. The preservice charge could be all or part of a remaining deductible, for example, or a sizable percentage of what the visit or treatment might cost. Those deductibles go up when hospital prices, drug costs, and labor expenses increase, as insurers try to slow premium growth by shifting more costs to patients.

    People are “basically being asked to self-insure,” said Richard Gundling, a senior vice president at the Healthcare Financial Management Association, an organization for finance professionals.

    As that happens, hospitals figure more patients will have trouble meeting those deductibles, so they want to get as much up front as possible.

    “Things like preservice deposits and those kinds of moves are probably going to become more and more likely,” said Chip Kahn, a visiting senior fellow at KFF and the American Enterprise Institute and former president and CEO of the Federation of American Hospitals. “That will make it harder on the provider, the clinician, and harder on the patients.”

    The deposits can’t be viewed in isolation, Gundling said: It’s a bigger issue than just hospitals asking for money up front. The challenge, he said, is: “How do we maintain access to care when more patients can’t absorb the level of out-of-pocket costs?”

    Already, consumers are increasingly worried about paying for healthcare. A recent KFF health tracking poll found that lower out-of-pocket costs ranked as the top change insured adults would like to see from their coverage plans. KFF is a health information nonprofit that includes KFF Health News.

    The average deductible in family coverage offered by employers is $3,762 per person, according to KFF, while the average deductible in Affordable Care Act plans jumped 37% this year to a similar amount, $3,786.

    A consumer concern

    Community Health Advocates, a health insurance consumer assistance program in New York state, hears from people who are concerned about prepayments, said Diane Spicer, a supervising attorney.

    “We see this mostly with insured folks who are seeking out-of-network care but who have out-of-network coverage,” Spicer said, “and also sometimes for care that is not covered.”

    Just how many hospitals collect what are often called point-of-service payments is not known, according to Kodiak Solutions, a technology company that provides services to health systems to help manage their revenue.

    “But it is becoming more and more the center of many of our conversations with health systems,” said Matt Szaflarski, a vice president leading Kodiak’s revenue cycle intelligence team.

    In addition to Mayo, Baltimore-based Johns Hopkins Medicine’s website says that “it is our policy to collect all amounts owed before services are rendered” for non-emergency care. University of Texas-affiliated MD Anderson in Houston, one of the nation’s premier cancer treatment centers, says patients who pay for their own care “will be asked to pay an initial deposit determined by the care center, based on the type of cancer.”

    On average, hospitals collect about a quarter of what they expect the patient will owe, Szaflarski said, based on what they estimate the insurer will pay — a percentage that has grown in recent years.

    For example, if a person is coming in for imaging and the insurer will reimburse $1,000 for that scan, the hospital will seek $250 from the patient up front, he said. “That used to be closer to $150.”

    It also varies by hospital, and sometimes by state.

    “The state of Indiana has some of the lowest cash collections in the country. They are Midwest nice,” Szaflarski said. He added that California and Texas are among those that collect more.

    Even as hospitals increasingly collect more upfront payments, however, their uncollected debt is also rising, according to data Kodiak collected from more than 2,300 hospitals nationwide.

    A Kodiak report in June said that’s because of a “fundamental shift” in coverage as plans “increasingly feature higher deductibles, greater coinsurance, and more complex cost-sharing structures: all elements that increase the nominal patient responsibility without improving—and often reducing—the probability of collection.”

    While many hospitals are doing fine, some, especially in rural areas, have thin margins — and things could soon tighten further as cuts to ACA and Medicaid funding lead to more people being uninsured.

    As a result, hospitals “have to be concerned” about every cost-sharing dollar, Kahn said.

    After Zordani returned to Denver, he said, it took a while to find another specialist. He eventually had a procedure in late June 2024, at a Denver hospital not affiliated with Mayo, to fix a spinal fluid leak.

    The following fall, he filed a complaint against Mayo in Arizona civil court. He was awarded $47,500 in economic damages and attorney fees after an arbitrator in September 2025 determined Mayo violated a state consumer fraud law because it failed to reach him to say that his plan was not in-network before he traveled. Mayo’s statement to KFF Health News did not include any reference to the settlement.

    “Had they notified me in timely fashion as required, I would not have flown there,” Zordani said. He’s still angry that the clinic didn’t ask his permission before designating his care as self-pay, which meant he wasn’t going to use his insurance, and he’s still unclear on how they calculated the $5,000 preservice amount.

    When do consumers have to make preservice payments?

    There is one clear rule: In emergency situations, hospitals that accept federal Medicare financing cannot, by law, demand upfront payment before stabilizing a patient who arrives at an ER, said Matthew Fiedler, a senior fellow and health policy researcher at the Brookings Institution.

    Other consumer protections are less clear.

    Patients who get in-network care may have some recourse in their contracts with their insurers, so they should check the fine print, experts told KFF Health News.

    “In out-of-network settings, I’m not aware of any barriers that would prevent a provider from doing this,” Fiedler said of preservice deposits.

    How those amounts are calculated also appears widely up to the provider and can be opaque.

    “They could just say $1,500 and you’d be like, ‘Oh, is that 10%, or is that how much is left on my deductible?’” said Patricia Kelmer, senior director of healthcare campaigns at PIRG, a national federation of independent consumer advocacy groups.

    Yet, she added, the patient might be scheduling three months in advance, so the provider wouldn’t know how much was left on the deductible. She recommends consumers ask for an itemized bill and call their insurer to find out whether it has rules regarding the charges.

    Also unclear are how and when patients get their money back if they overpay.

    Overpayments can happen if patients don’t require the services originally estimated or when insurers pay other bills first, such as the anesthesiology cost or a surgeon’s fees. If those payments are counted toward a patient’s deductible, yet the patient had already made a prepayment to the hospital for the expected deductible, they’ve now paid too much to the hospital.

    How soon they get their money back can vary and can depend on state laws, though a small number of states directly address the issue. As of this year, Florida requires medical providers to reimburse patients within 30 days of a determination of an overpayment. Some states, including Maryland, prohibit certain hospitals from requiring prepayment simply to avoid offering financial assistance.

    After alleging that some patients had to wait more than a year to get reimbursed, Arizona Attorney General Kris Mayes recently brought a suit under state consumer protection laws against SimonMed Imaging, which has 170 locations in 10 states.

    In a settlement, SimonMed agreed to issue refunds within an average of 60 days.
